How we work this out
For off-label ketamine — infusions, injections and lozenges — we take the published price band for Indiana and multiply by the number of sessions in the course you picked. Coverage does not change that figure, because these are not FDA-approved for depression and are almost never covered.
For Spravato we start from the drug's list price and a typical administration fee, then apply the rules of your coverage type — Medicare's 20% coinsurance, a commercial plan's coinsurance widened by whether your deductible is met, or the manufacturer's per-dose figure where the savings card applies.
Where we cannot be honest, we do not give a number. Medicare Advantage copays, Medicaid approvals, VA referral pathways and self-pay Spravato prices all vary more than any estimate could capture, so we say so and tell you who can answer instead.
Every figure is rounded outward — per session to the nearest $25, course totals to the nearest $100 — so that nothing here can be mistaken for a quote. Ranges are never narrower than about 1.4 times the lower bound, because a tight range would imply a confidence we do not have.
Our biggest known weakness: Spravato's list price is not published by the manufacturer or in any free public source. The figure we start from is a reported launch price from 2019 and is very likely low. That is why Spravato estimates are labelled rough, and why we refuse to produce a self-pay Spravato figure at all.

Why the range is so wide
Clinics set their own prices and few publish them. These are the things that actually move the number, and every one of them is a fair question to ask before you book.
- Whether the clinic bills insurance at all, or is cash-only.
- Whether an anaesthesiologist or nurse anaesthetist is present for the infusion.
- Where in Indiana you are — metro clinics generally charge more than rural ones.
- Whether the clinic sells prepaid packages, which lower the per-session price but tie up your money.
- Whether the first psychiatric evaluation is included or billed separately.
- Whether the two-hour observation after Spravato is a separate charge.
IV ketamine: the cash-pay reality

Because it is off-label, IV ketamine is essentially never covered by insurance — so for most people the price above is what they will actually pay, in full, themselves.
UnitedHealthcare's national policy states that ketamine injection is investigational, and therefore not proven or medically necessary, for psychiatric conditions including depression, bipolar disorder and post-traumatic stress disorder. Many clinics offer packages at a lower price per session. Those packages are usually prepaid, which is a real risk: if the treatment does not suit you, or you cannot tolerate it, you may have paid for sessions you will not use. Ask what happens to the balance before you pay.
Spravato: the insurance route

On 17 January 2025 the FDA approved Spravato for treatment-resistant depression in adults either on its own or together with an oral antidepressant, so a companion oral antidepressant is now optional for this use — though it is still permitted. as of 17 Jan 2025 · U.S. Food and Drug Administration (source 3, opens in a new tab)This applies to the treatment-resistant depression indication only. The separate indication for major depressive disorder with acute suicidal ideation still requires an oral antidepressant — see spravato.approval_mdd_si.
